Visual field testing is a measurement of areas of vision seen both centrally and peripherally while fixating on an object. Perimetry or campimetry is one way to test the visual field by systematically measuring differential light sensitivity in the visual field by the detection of test targets on a defined background. Perimetry more carefully maps and quantifies the visual field, especially at the extreme periphery. Static perimetry uses fixed targets while kinetic perimetry uses moving ones.
There are several methods for performing perimetry. Visual fields by confrontation is a method in which the patient covers one eye and identifies how many fingers the examiner is holding up in different quadrants of the visual field. The simplest form of perimetry devices uses a white tangent screen and presents different sized pins attached to a black wand, which may be moved, against a black background. The pins may be white or colored. The use of a white background and lights of incremental brightness is called "white-on-white" perimetry and is the most commonly used in clinical practice for the early detection of blind spots and research trials where loss of visual field must be measured. However, the sensitivity of white-on-white perimetry is low and as many as 25-50 percent of the photoreceptor cells may be lost before changes in visual field acuity are detected. The Goldmann perimeter is a hollow white spherical bowl positioned a set distance in front of the patient. An examiner presents a test light of variable size and intensity, which may be fixed of move towards the center from the perimeter. The Goldmann method is able to test the entire range of peripheral vision, and has been used for years to follow vision changes in glaucoma patients. More recently, automated perimetry has become common. Humphrey visual field is an example of automated, static perimetry that measures the level of light sensitivity within the field of vision by adjusting the intenisty and size of a target illumination. Automated kinetic perimetry uses a mobile stimulus that is controlled by a perimetry machine, with the patient indicating whether he sees the light by pushing a button. Source: Regenstrief LOINC
